Brief Answer:
I think that surgery at L3-L4 level is necessary.

Detailed Answer:
Hello and welcome to "Ask a Doctor " service.

I have read your query and here is my advice.

There are several levels of your spinal cord that are damaged by degerative changes, and protruded discs at the thoracic and mainly lumbosacral part.

This according to MRI report that you uploaded.

The complete stenosis at the L3-L4 level and the spondylolysthesis, in my opinion, should be treated surgically ( decompression and stabilization ) because of the potential paralysis that it may result ( if there is no partial paralysis of the legs and problems with urination already ).

The aim of the surgery in such case is to improve the symptoms and to prevent progression of the condition.

Hope you found the answer helpful.

Let me know if I can assist you further.

Thanks a Lot Sir. My further queries are as under:

What is the risk involved as per the age of the patient.

What is the recovery period for such surgery.

Do you think that the surgery at L3-L4 level will relieve the pain and will help the patient to walk; though there being compression at different levels of the Spine.

Required your opinion on the said aspects.

Brief Answer:
Decompression of L3-L4 should improve significantly the symptoms.

Detailed Answer:
Welcome back.

Actually there are some risks that should be considered and accepted with this type of surgery.

These risks consist on the previous health issues ( for example cardiac or pulmonary diseases ), possible complications of anesthesia ( very rare ), failure of surgery ( rare), post surgery infection ( very rare ) , etc.

According to MRI findings, the L3-L4 level seems to be the culprit, this should be correlated with neurological examination findings.

The decompression and stabilization at this level outweighs significantly the possibility of failure and should improve all the symptoms.

Other levels seem not to cause significant neurological symptoms according to MRI findings.

In case of success recovery takes few weeks.

Hope this helps.

Take care.

Brief Answer:
There is stenosis above and below L3-L4.

Detailed Answer:
Hello again.

I examined the MRI images that you uploaded.

I see that there is significant stenosis also at the levels above an below L3-L4.

In my opinion, surgery is necessary and, in this case, posterior laminectomy L2-L3-L4 with stabilization seems to be the appropriate approach.

Hope I helped you.

Take care.
