My symptoms are:
weight loss - 2.5kgs in a month
hair loss
mild fatigue
No other known disease. Also attached USG Thyroid for you reference.

Weight loss is the most usual symptom of hyperthyroidism and it can be correlated with your thyroid test results.
YOur thyroid ultrasound show enlarged thyroid but the structure of the gland is normal.
Considering your test results and your symptoms i can deduct that you have Graves's disease.
I suggest to consult physically a local endocrinologist because it is necessary to start the treatement with antithyroid medications (methimazole or propylthiouracil).
These are not OTC medicines and may have adverse effects for this reason my advice is to consult your local endocrinologist for the treatement and the follow up.
