Background: 1. In November 1, 2019 I Had Unprotected Sex

Question: Background:

1. In November 1, 2019 i had unprotected sex with my girlfriend. She suspected urethritis at the time but now in retrospect maybe it was a UTI.

2. November 13th. I got strep. Took amoxcillin for 10 days.

3. November 17th after having sex 3 times in 1.5 days after the last ejaculation i did not release sperm. And i felt pain. The next day was a sudden onset in my testicle of pain. It was hanging lower and swollen.

I have taken the following test since then:
A. Ultrasound of testicles
B. Blood test (CBC)
C. Chest x Ray
D. Urinalysis and std test
E. Urologist did a physical exam of the testicle and pelvis.

Only thing noticed was a 2mm epididymal cyst. Doctor said that was not of concern.

Now 1 month later my symptoms remain as followed:
1. Tender testicles
2. Swollen right testicle that hangs.
3. Aching in the testicles and pelvis
4. Frequent urination at night

My questions are what additional tests can i take? What could this be? What could alleviate the pain?

Did the doctor do a rectal/prostate examination? If so, and if you felt exceptional pain during this examination, it supports prostatitis.

What exactly did the urinalysis say?

Urinalysis showed leukocytes slightly above normal. Every other category was in the normal range.

I got a rectal exam last year and not this year. That exam was normal. He stated i had urethral spasms that manifest with some prostatis like symptoms.

What i don’t understand is why the swollen testicles?

Another key point to consider is that the swelling comes and goes. The testicles are always tender but the swelling may last for 3 days and then goes to normal for 2 days and then swells back up.

There is no rhyme or reason to it or anything abnormal i am doing on the days the swelling goes down

Additionally i read something about the mumps causing testicle swelling. Is there a test for that? And any likelihood of that being a culprit? How long would it last?

A blood test for mumps virus antibodies can be useful

I will encourage you to get an urgent ultrasound the next time your testicle swells, so that the cause can be found.

Mumps virus can cause orchitis (testicular infection). This can be tested from PCR methods of urine or a blood test for mumps antibodies.
