Dear doctor,
My wife is 70 year old and is having type II diabetes since 2008. Her glucose levels are well withing the normal limits and Hb A1 c is 5.8.IN the year 2014 one doctor suggested her to take 75mg of Ecosprin , though she has no history of heart disease or any heart problems. Yet she has started taking the ecoswprin till now. recently one doctor said, it is not necessary to take asprin for the people who has no heart disease. Kindly advise. I f you wish to know more info please inform i shall provide. Her lipid levels are all normal.
Cholestrol 129 tryglicerides 128 HDL 52 LDL 53 and vldl 24 as on April 2019. mg/dl

I have gone through your query and here is my opinion.
As per XXXXXXX Heart Association guidelines,all those diabetics who are at high risk,should be on regular low dose aspirin for primary prevention of cardiovascular disease.
But aspirin recommendation is based on a patient’s individual health status,family history,associated comorbidities like hypertension and hypercholesterolaemia,significant family history,control of diabetes,medication history and body profile.
Different doctors can have different opinions regarding the same patient.
I personally think that in the absence of a significant cardiac history and in context of an advanced age (where risk of bleeding outweighs the cardiac benefits) and a good control of diabetes and lipid profile,it's better to avoid aspirin.

Dear doctor i thank you for your guidance.as far as her cardiac history she is absolutelu perfect and no cardiac history except mild hypertension generally 120/80 and sometines 140/80.she was never refered to a cardiokogist so far in her life.pl advise

This would help me to evaluate her cardiac status.
As she is an elderly lady with tight control of diabetes/hypertension,has no cardiac complaints, no significant family history, so according to my clinical experience,she should not be on routine aspirin prophylaxis.
There is always an increased risk of bleeding in elderly patients.
Aspirin prophylaxis should not be used routinely as a blanket regime and should be individualized and used only for high risk patients.
