Question: CHRONIC CONSTIPATION
Dear Doctor,
This question relates to my wife. She is 68 year old. she is totally a vegetarian .She has chronic constipation since many years. Doctors say , drink plenty of water, take fibre, eat fruits etc etc.BUT despite taking all these, the problem is still persisting. Defecation takes place only after 3 to 4 days to occur. She at times take duphalac some times and every day she takes Wonder Laxo (an ayurvedic medicine comprising sunamukhi , karakkaya, tegada and saindhavalavanam)

She is diabetic since 2005 . Her diabetes is fully under control. Hb A1c is 5.8, Fasting 110 and Post lunch 125. Her lipid profile is under Normal levels She takes GLYCOMET 500 MG TWICE A DAY, Ecosprin av 75 one per day.

She has Hpothyroidsm since 2012. Her TSH is 1.8.She takes thyronorm 50 mcg per day

She has mild hypertension since 2000 which is well under control .Its 120/80. she takes Stamlo 5 mg

She takes Tab A to Z in the lunch and Beminal after dinner every day.

I know diabetics will have constipation with all these medicines. She has this constipation even before she has diabetes, thyroid and BP.

Kindly give me a solid solution for this problem. I am vexed hearing fibres, water, fruits etc . In your opinion what is to be done to have normal and regular soft defecation. she has no stomach pain at any time..she often complains about belching and some times bloating. Thats all.

She takes 2 britannia biscuits every day she takes normal vegetarian food , and for dinner she takes 2 pulkas. .she takes lemon water with je every day in the morning. At times she takes ginjer water.

I have gone through your query and here is my advice.

As per your history, constipation is side effects of tab A to Z and tablet beminal. These multivitamin tablets may cause constipation. So I think you should discuss this with your physician. Still kindly follow up with a bit more information.

Please let me know does she have history of weight loss, pain or bleeding while defecation, decreased appetite?

Kindly strictly avoid biscuit and other bakery items. You can also take alternative laxative (fiber supplements) containing psyllium or stool softener like docusate sodium in case of hard stool after physician consultation. Do regular TSH level monitoring.

Do regular exercise for 30 to 40mins like brisk walking. Avoid heavy meals. Keep your self hydrated. Continue with diet containing fruits, vegetable salad, whole grain items.

Still if not getting relief, for further assessment she may require Colonoscopy, complete hemogram after detailed medical examination and gastroenterologist opinion.
