default
Posted on Tue, 5 Jul 2016
Question: Hi doctor, i had unprotected oral sex with a call gal for 10 sec she sucked my penis, after that we hav naked fortage for 2 min(no condom) and no penetrative sex (neither vaginal nor anal sex) i m worried if their
might be any vaginal fluid on her vagina and during naked rubbing of body that fluid may have came came in contact with my opening tip of penis, is their any risk for Hiv from this exposure,
i had hiv rna 1 and 2 pcr test on 13 day(qualitative) and hiv duo test on the same day(13 th day) result came back as not detected for hiv rna 1 and 2 and negative for hiv duo test
wbc count on 13 day 8100 and on 15 day 8600
so my question how soon is hiv rna detected and how soon p24 is present in the body after exposure
can i conclude my self negative is p24 can be detected on 13 day.

Brief Answer:
HIV RNA test is more conclusive after 3-4 weeks.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
Oral sex carries a low risk for HIV when compared to anal and vaginal sex. HIV transmission risk for frottage is negligible as per CDC XXXXXXX United States.
Total WBC count between 4000-11000 is normal. So your WBC count is quite normal.
HIV Qualitative RNA PCR can detect the presence of HIV 1 infection as early as one week. But it is considered as more conclusive after 3-4 weeks. P24 antigen also can be detected by one week in the presence of HIV infection.
Usually PCR test can detect either HIV 1 or HIV 2 with separate test kit. Have you had your tests for both HIV 1 and 2 separately? Then these tests almost ruled out both HIV 1 and 2 infection.
These tests are 99% reliable after 3-4 weeks. If you have a test with antibody 1 and 2 detection test or 4th generation test 12 weeks after the risky exposure it will be 100% confirmatory.

yes i had done 2 different test for hiv 1 and for hiv 2 also(rna pcr) is it possible that test wont detect the virus after 13 of exposure, done ma test at SRL lab many website say hiv virus will be high in 10 days and it will give positive if ur infected so how much percentage it is conclusive at 13 days m a healthy and normal guys so if i would be infected the viral load will be high , so how much conclusive i can be at this time

Brief Answer:
no constant facts in medicine as with mathematics.Differs with persons.

Detailed Answer:
Hi,
Welcome back.
What you have mentioned about the viral load. It will be high in early seroconversion period or Primary HIV infection. But it may vary slightly in situations with certain individuals depending upon the inoculum, response of the body to the virus to get established from skin or mucus membrane to reach the blood stream. This test had a window period of 10-12 days and has an accuracy of 90% at 10-12 days, 95-98% at 6 weeks, and 99% at 12 weeks.
First detection of HIV RNA: approximately ten days after exposure (typical range 7 to 21 days).http://www.aidsmap.com/page/0000/
I hope that you would have convinced with these data.

hello sir one last question who are this people in whom rna will be detected at later stage and wat is inoculum basically in how many percentage of people will have delay in production of rna, and basically how many days does hiv take to enter blood stream from wet mucous membrane
doctor
Answered by Dr. Sankaranantham Murugan (2 hours later)
Brief Answer:
HIV enters thru' skin/mucus membrane take nearly 72 hours to get establishe

Detailed Answer:
Hi,
Welcome back.
One who acquires HIV through blood transfusion or a deep prick contaminated with HIV positive blood (in hospitals and labs) will have very high viral load faster than with whom acquires through sex contact. Breast milk and saliva are having very low concentration to cause an infection. Moreover the travel of virus from skin or mucus membrane to blood cells will take around 72 hours normally whereas certain host factors like immune cells like macrophages and dendritic cells and certain giant cells may delay the appearance of detectable quantity of antigen in the blood for some more time with certain individuals. Apart from these certain unknown and unexplained factors may play a role.
Exact percentage and all not explained by anybody so far.

Brief Answer:
These findings alone have no specific value. Normal variations

Detailed Answer:
Hi,
Welcome back.
Variations in blood cell count in normal individuals are not uncommon. This will remain for sometime and once again settle to normal. These mild abnormalities of blood cells count alone have no specific value. Along with the other investigations and findings only we can arrive at a conclusion.
Not to worry about the same.
Your HIV PCR tests are reliable.
