Question: Crohns Dx patient 90% remission if skip cananasa for while or miss humaria will be symptomatic
- take humaria weekly, asacol HD 800mg and cananasa and nexium. I had to switch GI because my doctor left the state. My current doctor doesn't examine me or anything just ask if I need refills. I have decided my pcp can follow me better than the GI. My question is what all should he do in his exam, how often should I see him and what labs or test should I have and how often

Hello sir,
Firstly you should not stop humera and asacol at any cost until your doctors advice, always your doctor should examine your stomach to check for the tenderness and also gain in weight

Next thing about how often I need to see your doctor: initially monthly once for at least couple of months and later once in 3 months if no symptoms are there and if you are doing well once in 6 months will be good .

Regarding labs : initially complete blood count,ESR ,CRP,SERUM CREATININE,VIRAL MARKERS ,ULTRASOUND ABDOMEN .Later if u are visiting once in 3 months Complete blood count ESR,CRP,serum creatinine will be fine.And if symptoms are still present even after 6 months a repeat colonoscopy should be done.

When you are on humera there are quite a few side effects that you should be cautious and call your doctor immediately like, difficulty in breathing, itching, joint pains, rashes, vision problem.

How often should have general rectal exam since crohns in rectal area and how often do I need a colonoscopy while in remission or almost remission.

I know I should never stop the canananas I just don't like the med due to it being a suppository. I try spreading it out a few days and then don't have issues and keep going till I put the line too far.

Crohns usually doesnt involve rectum and we usually call (rectal sparing) and if you are in remission then usually untill there is a flare ,no need of any colonoscopy .
But even on therapy still symptoms persist then definitely you need to get a colonoscopy and biopsy to know the reason .
If you are really doing well then as a generalized screening to compare the lesions from previous initially 6 months and later over a period of 1-2 years will be fine.
And can you discuss the colonoscopy report if you dont mind ,and what does canananas contain that you are using as a suppository..

Canasa rectal
I was having rectal pain and some bleeding and then they saw some small ulcers in the anal/rectal area not sure. If I go off this more then 2 weeks at time start having issues. That was the reason I was asking how often a DRE or anoscope in office was needed for monitoring.

My crohns is in the TI and colon area. Was dx in 2002.
Confused about what meaning could you discuss the colonoscopy report

if you are not having any pain and totally reduced symptoms then your doctor might consider to do a colonoscopy after 3 months just to check the inflammation ...and you can check with your doctor he can consider a foam too if you are not comfortable with suppository.
And regarding the report which I asked to discuss is because I just wanted to know which part of colon is involved.

Thank your for taking my question serious. I know it seemed strange but I had a battle with Crohns when I was first diagnosed and was seeing doctors all the time for almost two years. I am for the most part in remission as long as I continue my medication regime. I take the humaria every week, I take 800mg of Asacol I was on 1g of Pentasa but they came out with 1 pill Asacol and my pcp asked me since I was in remission for the most part would I would I like to try this and it would only be 1 pill a day vs 2 twice a day. I am suppose to take the canasa every night or at least every other night. I just don't like it, it takes time to administer not as easy as swallowing a pill then going to bed.
My first GI I ever saw was at XXXXXXX Clinic and then moved to Vanderbelt. I saw him on a yearly basis and my PCP managed everything. Every one of his exams was just about the same, he would start at the eyes, throat, neck, thorough abdominal exam, check ankles for swelling, ask about rashes, joint pain and would end with a rectal exam and check stool for blood. He always did blood work. I got it in my mind that this is how all my exams would be. Now that they are not it makes me uneasy we are going to miss something or we could catch something before it is too late.
Summary
Here is what I think I understand from your answers.
1. Even if in remission still need follow-up every 3 months or at least every 6 months
2. Complete blood count ESR,CRP,serum creatinine every three month
3.Dr needs do do an abdominal exam and monitor weight a rectal exam is not needed every 3 months maybe once a year
4. Don't stop my medications without the doctors say. Even the canasa. I did the Rowasa enema and hated it even though it helped.
5. Keep my doctor informed of any new symptoms

Question
If crohns does not target the rectal area why am I am the canasa, and have rectal pain, and some bleeding when not taking it as should.
