Posted on Tue, 4 Jul 2023
Question: I received unprotected oral on sunday evening. I've never done it before. I ejaculated into a hand not the giver's mouth. Mouth went around my member and there was licking. There was some precum during the process. I am scared of a std and or hiv!! I tried to get tested at a clinic today but need to wait 2-3 weeks? I am seeing my doctor next week.

Brief Answer:
You have a very low risk for HIV/STD.

Detailed Answer:

Hi,

Oral sex carries a low risk when compared to blood transfusion, sharing of needles, needle stick injury, anal and vaginal sex. Human saliva is not infectious for HIV unless it contaminated with blood as per CDC, USA.

Your pre-ejaculatory fluid has nothing here. HIV PCR, P24 antigen tests can detect HIV 1 infection as early as 1 week, but become more conclusive after 3 weeks. Common tests for HIV/STD are more reliable after window period (6-12 weeks). They can rule out both HIV 1 and 2 and STD as well.

When I see the doctor next week, should I raise the antigen test or wait longer? I still wish to get tested for other stds as well.

What do we mean by low risk? Is there a still a chance of infection? What about other stds?

I have masturbated using a pillow for several years leaving my penis glans sensitive. Is there any way HIV or another STD enter the penis glans or urethra?

Brief Answer:
Tests 3 months after the exposure are more conclusive.

Detailed Answer:

Hi,

Masturbation with a pillow has no impact on the risk of acquiring HIV /STD. Vaginal sex is considered as high risk and has a chance of 5-8 /10000 exposures. Oral sex carries a still lower risk.
You can wait till 3 months and can have the tests to rule out HIV 1 and 2 infection and STDs. That will be more conclusive.

What about other stds such as gonorrhea? When can I get tested for them?
doctor
Answered by Dr. Sankaranantham Murugan (8 hours later)
Brief Answer:
Gonorrhea is unusual through oral penetration sex.

Detailed Answer:

Hi,

STDs like Gonorrhea and Chlamydia can be a risk for the recipient (one who uses their mouth for oral sex), not to the inserter. It is a very rare possibility for a penetrator to acquire Gonorrhea or chlamydia through oral sex.
If you don't have symptoms pertaining to urethritis (burning sensation during urination and urethral discharge), you need not bother about Gonorrhea.

What herpes?
I had a preliminary blood test for STDs today to see if I was clean before Sunday.
Will blood work and urine reveal anything?

I have had protected sex twice and now concerned if I got herpes. I read they can appear on one's legs such as the thigh?
doctor
Answered by Dr. Sankaranantham Murugan (1 hour later)
Brief Answer:
HSV can occur rarely over thigh

Detailed Answer:

Hi,

Herpes simplex infection is caused by HSV 1 and 2. HSV 1 is usually transmitted through non sexual route around mouth and nose.
But it can manifest over genitals through oral sex. Herpes simplex infection can rarely manifest over thigh and buttocks also.

How can HSV 1 manifest over genitals? What symptoms indicate its presence?

What about HSV2?

I got some prelimary bloodwork done today to determine if i had any STDs prior to Sunday. Without a existing std, what are the chances of contracting HIV through unprotected oral?

doctor
Answered by Dr. Sankaranantham Murugan (8 minutes later)
Brief Answer:
Oral sex carries a low risk for HIV/STD.

Detailed Answer:

Hi,

Both HSV 1 and 2 manifest in the same way with groups tiny vesicles. They will rupture to give rise to grouped tiny ulcers. It is a self limiting disease and ulcers will heal within a week or two. In the absence of STDs, risk of HIV transmission through oral sex is remote.

I've noticed a slight discomforture while sitting down and redness near the tip of my penis. Is that related to Herpes?

I forgot to add, I had protected sex twice in my life. How effective are condoms against herpes?
doctor
Answered by Dr. Sankaranantham Murugan (47 minutes later)
Brief Answer:
Condoms will reduce the risk of transmission

Detailed Answer:

Hi,

Your symptoms are unlikely to be due to Herpes. Condoms will definitely reduce the transmission rate of Herpes to a considerable extent.

Hope I have answered your query.
