PCOS run a high risk of developing diabetes in the future. Metformiin is recommended in PCOS if the patient has glucose intolerence ( by an oral glucose tolerence test) or diabetes.
However, many endocrinologist ( including myself) prefer Metformin in PCOS which helps to regularise the periods by reducing the insulin levels. However, this is an offlabel treatment. If used off label to reduce insulin levels, what dose is appropriate?

The maximum efficacy is seen with 1500 mg daily. However, not many patients can tolerate this dose. If you are obese, then this dose is okay. Otherwise, start on 500 mg once at night and over a period of time make 500 mg twice a day. Keep at that level for now
