There is no problem in using sodium bicarbonate pills in a patient with advanced kidney disease.
In fact from CKD stage III onward we check the acid levels in patient's blood and if these are high we prescribe them oral sodium bicarbonate pills.
In some studies this medication has been found to be helpful for retarding the progression of kidney disease as well.
