Can Thyronorm And Glycomet Be Taken Together?

Question: My 16 year old son is borderline hypothyroid- TSH-4.95. Was taking thyronorm 25 mg. TSH came down to 3.85. He has been having persistent daily headaches for more than 6 months. Migrain Medicines do not seem to help these headaches.Doc has asked him to stop thyronorm and start Glycomet. He is Obese.Is Glycomet for weightloss? Isn't TSH 3.85 also high? I believe TSH should be in the range 0.3-3? Can Thyronorm & Glycomet be taken together? Can headaches be due to thyroid problem?

He does have most symptoms of Hypothyroidism like -overweight, lack of concentration, dry skin, slurred speech etc.

Should he continue with Thyronorm as TSH is in control by Thyronorm (to 3.8 from 4.95)? Why has the doc suggested Glycomet instead of Thyronorm?

Hi Sir
i have carefully read your query and understand your concern.

A Tsh value of 4.95 is not considered as boderline but well defined hypothypothyroidism.

Normalization of TSH to reference range take about several months.

Dose changes of thyronorm should be made every 8 weeks until TSH is in the target range.

After dose stabilization the patient can be monitored with annual evaluation and TSH monitoring.

The complete symptom list of hypothyroidism take about several months to improve.

Based on these scientific approaches,stopping Thyronorm for values 3.8 is not advisable in my opinion.

It should be continued until ode stabilization is achieved.

Even Headache can be manifestation of over treatment of Hypothyroidism(but not due to hypothyroidism per),I do not attribute this to be the cause in your child's case.

Your Physician might have stopped Thyronorm taking into consideration the symptom of headache and TSH value being close to normal.
But this seems to be irrational and unscientific.

Sudden stoppage of Thyronorm can cause rebound increase of TSH levels and precipitate myxedema coma(a severe form of hypothyroidism).

Please note that your son is not completely cured of hypothyroidism and he still needs treatment with Thyronorm for some more time.

Metform(Glycomet) is not for weight loss.This drug is an antidiabetic/sugar lowering drug known to improve insulin resistance that occur in obese people.

This drug can be used in your case to improve the metabolic abnormalities in your son's case and hasten biochemical recovery from hypothyroidism induced biochemical abnormalities in the blood.

Please note that Metformin is not a substitute to Thyronorm inspite of above advantage.

Headache in your child's case has to be assessed further through a non contrast CT scan of brain for ruling out other significant causes of headache.

-What is the ideal TSH range?
-He has got a non contrast MRI of brain done. Did not show anything wrong. Is CT scan of brain still required?
-Is it OK to take Thyronorm and Glycomet together? (Thronorm 25 mg in morning & Glycomet 500 mg with breakfast and Glycomet 500 mg with dinner)

The normal TSH range is 0.3-3iu/ml.

Non contrast MRI is more sophisticated and informative than CT scan.
Hence a normal MRI does not warrant further CT scan.

Yes its very much okay to use both thyronorm 25mcg and glycomet together.

But it would be better if dose of glycomet can be 250mg twice a day ,since low blood glucose levels caused by glycomet can precipitate headache.

Have a periodic monitoring of blood glucose every week to detect ant unwanted hypoglycemia.For this maintain a glucometer at home.

I also recommend oral Vitamin B12 supplementation 500mcg a day as glycomet is known to cause significant B12 deficiency.
