Question: Hi.

My ORL doctor give me some medications for my nose issues. The list is bellow:
Vimovo
Borenar
Nasonex

My psychiatrist is on holiday and is not answering his phone. Do any of these have any issues with my Cipralx ( 10mg/day ) medication?

Brief Answer:
Significant interactions may cause trouble. You need alternative!!!

Detailed Answer:
Dr. Hanif warmly Welcomes you!

Thanks for consulting at my virtual clinic. I have carefully worked through your case, and I can realize your health worries. Being your physician, I assure you not to worry as I will take care all of your medical concerns.

Suggestion:-
You are good to take Borenar (Bilastine) but you clearly need an alternative to Vimovo.

Reason:-
I usually don't suggest to combine the Vimovo (naproxen sodium/Esomeprazole) with Cipralex (Escitalopram).

1. It is because Esomeprazole and Escitalopram interacts in a way that Esomeprazole increases the blood levels of Escitalopram by affecting its liver metabolism (that is for the excretion of Escitalopram from the body). So it will be as if you are taking more than the usual dosage of Escitalopram and the effects may be serious.
If it is needed to be given together for any reason a very close monitoring is needed which I never suggest in the case that your Psychiatrist is on holidays.

2. Naproxen sodium is the second ingredient in Vimovo and has clear interaction possible with Escitalopram. It is known as synergistic effect in which an outcome is enhanced due to the two different drugs. Here the outcome may be the Upper Gastrointestinal tract bleeding. It is because Escitalopram affects the proper function of the plateltes and if given with naproxen may cause serious effects.

Remedy:-
For your specific case the best option is to take Ibuprofen/Famotidine combination instead of taking Naproxen/Esomeprazole.

It is necessary to avoid serious interactions aforementioned.

I still don't see if Nasonex is safe.

For any disease you have, a timely follow-up after sometime regarding the treatment efficacy and to see any need to change the treatment options is very necessary. Keep a follow up some days after this visit session ends.

Brief Answer:
No. Not a big issue with only one pill !!!

Detailed Answer:
I warmly Welcome you back with a follow-up.

No there is no issue with just a single dose as body has its own mechanism of adjustment for the derangements and little fluctuations in concentration of the chemicals.

Take a plenty of water and keep a good healthy diet. I pray for a good health of you and pleasant times ahead.

Thank you so much for your time. The orl doctor said i should take 2 pills of Vimovo per day. Should i do the same with ibuprofen? What concentration? Is Nurofen good? Is it necessary to take famotidine, as i don't usually have stomach issues, and i always take Nurofen only after eating.

Should i consider seeing a new Orl doctor?

Thank you again.

Brief Answer:
No need to change the doctor. You will get better with treatment.

Detailed Answer:
I welcome you back with a follow up and smiles extended alongwith the answer. It is an honor for me to be able to help you today for better health.

Well, Famotidine is not necessary if you don't have stomach issues. I usually give this in combination to the pain killers just to save them from stomach disturbances afterwards,

Yes, Neurofen is safe for you. You may take Neurofen Sinus Pain Relief 200mg/5mg Tablet twice daily after the meals. Rest of the instructions aforementioned are also important to be considered.
