How do you find out if it is cut. Is it X-ray or MRI or something else. When you said old how long did you mean after it was cut.

Clinical exam and MRI can tell you median nerve injury. Other test is nerve conduction study, but this is not a good test
Cut limit to consider old is mostly arbitrary. Usually it is one week

My wife had rotator cuff surgery on April 17th 2014. Since the surgery she has been in the same pain. It is her thumb, index finger and the one next to it. She said it is like a raw nerve and she cannot hold anything or squeeze those three fingers together but she has limited movement in them. We quite going to the surgeon that did the original surgery. We now have gone to another orthopedic Dr. who has sent us to a spine Dr. The spine Dr. has sent us to a pain management Dr. who has tried 2 times with a procedure thru her neck to try and stimulate the nerves by her spine. Originally this Dr. was going to do 6 procedures and now after 2 with no results at all she said she will do one more and if nothing works then she will try the next step to mask the pain. I think it is a pinched or severed nerve from the first surgery and the longer we wait the worse it could get. Should we now go to a neurosurgeon?

I think you need to see a neurosurgeon and physiotherapist for the treatment. This is not just a pain management case only.
Probably your doctors also wants to give pain modulator medicines like gabapentin etc
