Can Bilateral UV Reflux Grade Five Lead To Kidney Issues?
Question: Dear Dr.
Kindly comment on the prognosis for the case.
CHARAT SINGH/DOB: 3/10/16.
? Recurrent UTI since 6 months.
? Bilateral Vesico uretric reflux.
? Grade V on right.
? Grade IV on left
? Renal dynamic scan dated 15/11/2017 revealed differential function of 86 % in left kidney and 14% in right kidney (severely impaired functioning right kidney).

Hello,
Based on the facts and reports of your 18 months old son which indicate that he has bilateral UV reflux grade 5 on Rt side and Grade 4 on Left side resulting into the minimal function of the Right kidney with good function of the left side.
The prognosis in case of bilateral UV reflux is governed by the infection that spreads to the kidney and damage it causes to kidney.
In recent times there is an international protocol to observe these children with the recommendation of being given prophylactic antibiotic like (Sporidex)to your son to prevent them from getting UTI.
In most of the cases, VUR gets resolved over a period of 4-5 years as the child grows and can have normal life.
However, it is very important to monitor progress and see the status of VUR frequently at the interval of every month under the supervision of qualified Nephrologist by
1) Taking prophylactic antibiotic religiously
2) Doing routine urine analysis and urine culture (If required)every month
3) Doing DMSA scan at 6 months interval to assess the function of kidneys.
4) Report immediately to your Nephrologist if he gets fever, vomiting suggesting UTI
Your Urologist has rightly advised you the standard plan and modality of treatment.
