Question: hi doctor i have started taking zolfresh 5 mg because i have had panic attacks and had so much difficulty sleeping , i have not been able to sleep without the pills because when i dont take them i feel like i cannot breathe or i start to panic . i had visited doctors earlier and also have hyertension , i have seen a psychiatrist in XXXXXXX also but it didnt really help me and i stopped taking the medications .
i am married for a year now and now i want to start trying to have a baby also but also scared and want to give up zolfresh also because my doses i take has gone up to 20 mg also sometimes . please help i am
vey desperate and i am in nagaland .

I appreciate your efforts for medical consultation in so much distress.

From your description it appear that you are suffering from anxiety and panic disorder. Due to insomnia you have also become dependent on zolfresh.

Anxiety and Panic occur due to neurotransmitters imbalance in brain most commonly involving serotonin neurotransmitter. You need regular supervisied treatment with psychiatrist to treat your anxiety. It is not clear nature of treatment in past, but believe me people life changes after right treatment.

There is need for treatment, if symptoms cause significant personal distress or affect personal/professional life. Both drug and non drug treatment like cognitive behaviour therapy (form of psychotherapy) are used for treatment. Choice of treatment depends on severity of symptoms, personal preference and availability of treatment.

In drug treatment two types of medicine used. For immediate relief of anxiety benzodiazepines group of medicines (Like Clonazepam/Alprazolam) are used and for long term relief most commonly SSRI (selective serotonin reuptake inhibitors like Paroxetine) are used. SSRI takes around 3-4 weeks to show its effect.

All these process is complex. I have just simplified it to make you understand it. To follow treatment you need to meet some nearby psychiatrist personally. Medicines are also prescription medicines. It is illegal to buy or sell them without doctor prescription after evaluation, due to risk of addiction.

Apart from above advice following simple measures will help you:
-Decrease worrying by keeping yourself busy and distracted from worrisome thoughts by talking to family members or watching T.V. or counting back (100 to 0).
-Healthy diet: Milk and fruits in diet.
-Daily morning walk
-Breathing exercise, yoga and meditation (which you can learn from some expert in person)
-Blood testings: CBC, TSH, B12, Vit D levels
