Can Frequent Urination Cause An Itchy Scrotum?
Question: I have had itchy scrotum (worse in early morning and at night) together with pain when urinating and frequent urination for 4 weeks now. Went to pC, dermatologist, urologist. All exams (including STD - twice) came back negative. Application of ointments for itch have not worked, and antibiotics for possible UTI have not either. Only issue found was mucus in urine. Recently, slight pain in lower back has surfaced. Any connection between itchy scrotum and pain when urinating?

I am Dr kakkar (Dermatologist and Venereologist). I have noted your concern.
Itching on scrotum could be either due to a dermatitis or it could be Scabies.
Do you have any rash or visible skin lesions on your scrotum? I would also like to know the ointment that you have used?
Is there any history of risk exposure e.g unprotected sexual contact prior to the onset of burning micturation? Is there any associated urethral discharge?
I also request you to kindly upload a few clear images of the scrotum in "Reports Section" so that I am able to guide you better.

Thanks. No urethral discharge of any kind. Tip of penis burns, especially after urinating. Frequent urination with lots of urine. Ointments used include lotrimin, ketoconazole and desonide (I uploaded photos of last 2). No skin lesions on scrotum, but red rash. Skin on scrotum is very tough / scaly — before it was very smoothe. I will send / upload images. Itching / burning on scrotum, at times feels like something is “biting” scrotum (like when you get mosquito bite) , which brings more itching (especially in morning when I wake up). Symptoms emerged few days after sex with condom. I can upload lab report if necessary.

I have viewed the Images. You seem to have scrotal LSC (Lichen Simplex Chronicus). In LSC the affected skin becomes thick with prominent skin markings. Itching is the inciting factor for LSC and LSC in turn is itchy so it becomes a cycle: itch- scratch- itch cycle.
I suggest you to use a moderately potent topical steroid e.g either fluticasone propionate 0.05% cream (Flutivate cream) or mometasone furoate 0.1% cream (Elocon cream), twice daily for 2-4 weeks. In addition I suggest you to take a tablet of levocetrizine 5mg once daily for symptomatic relief from itching.
Burning at the penis tip is perhaps due to urethritis. I suggest you to take a urinary analgesic e.g syp. urilizer 1tsf with a glass of water, three times a day. Avoid using any soap etc on this area (specially on the glans penis) as soap bubbles can get inside the urethra and cause a chemical urethritis.
