Can Lipodystrophy Occur In HIV Negative Patients?

Question: Hi Doctors,

Could you please answer my doubt in lipodystrophy?
1. Will lipodystrophy occur in HIV negative patients? I have been tested 7 times since my last protected exposure before 18 months. All were negative. This includes PCR, DUO tests and Rapid Combo tests. I am feeling i am having lipodystrophy. Losing fat in buttocks etc..Have taken steroids in 2013 and one dose pack in Feb 2016
2.If it occurs kindly advice if doctors can diagnose the same. My lipid and liver profiles were normal when checked in March 2016.My PP Sugar is elevated in aug 2016. It is 160 in one hour after meal and 93 in fasting.

Let me know how to rule out lipodystrophy or any test required to rule out anything serious.

I understand your concern

Just be relax, It is not a serious issue.
Lipodystrophy can occur in the HIV negative patients.

1. Localised lipodystrophy can be occur due to repeated infection and pressure in that area, insulin resistance.
I would suggest to consult the MD physician,Lipodystrophy Specialist.
Skin biopsy, skin fold thickness measurement will help in the diagnosis.
Repeat the fasting and post meal blood sugar and insulin test.

Hello Doctor,

Thanks for answering my queries. I was slightly worried if my body is not producing antibodies yet. I have taken 4th generation tests but reading the below article

it says even after two years hiv antibodies doesnt show up. I also took multiplex pcr test as well which detect hiv1,2, hbv and hcv. All came back as negative.

Kindly advice.

Hello dear
Welcome back and nice to see you again
I understand your concern.

WHO recommends to repeat the test again at 90 days of exposure.
The 4th generation test or DUO/Combo test gives almost 95-99 % accuracy.
Combo test will detect the both antigen and antibody.
The antibody test gives 95% and
Antigen test gives 99 % conclusive result.
No one has reported becoming positive after this again at 90 days of exposure.

The window period for a 4th generation antigen/antibody test is four weeks.
You have undergone for tests 18 months after the exposure.
PCR, DUO tests and Rapid Combo tests detect viral load, antigen and antibody.
You have negative result for the all tests.
Antibody will produce in the body against the antigen.
But you have no antigen in the body then no question for production of antibody.
So just be relax, you are completely safe.
