Question: Can mild or some immunodeficiency in a 9 year kid cause a hiv 4th AB/Ag test to be a false negative? The test was done about 6 years after an unfortunate exposure. 
Or does it have to be severe immunodeficiency?

The answer to your question is no, there can't be any false negative reports in HIV Ag/Ab testing even in severe immunodeficiency. The following are few factors that are related to HIV exposure and infection :
1)direct contact like in sexual transmission
2)injecting the blood as in blood transfusion.
3)using same syringes as in intravenous drug abusers.
4)The amount of the body fluid exposed and the duration of exposure are important in transmission.
Antigen and antibody tests are very sensitive and they aren't false negative in immunodeficiency.
But there is a strain known as HIV- O strain of HIV -1 virus that gives few false negative results due to its biodiversity.
The presence of antibodies in the early samples can be confirmed by the results of the Vidas HIV DUO Ultra, the only test capable of discriminating reactivities between antibodies.
