Can My Mom At 60 Take The Covidshield (astrazeneca) Vaccine With Sarcodiosis/Neurosarcodiosis Condition?

Question: Hello Doctor,

My mom is 60 years of age and she was diagnosed with Sarcodiosis/Neurosarcodiosis few years back (symptoms - skin rashes, nose bleeding, lymph nodes swelling, headaches etc.)
She was treated and has recovered well. She however continues to take defcort 3mg every alternate day. She has been taking these medicines for over 5 years atleast.
My questions are

(1) Can she take the covidshield (astrazeneca) vaccine with her condition? Is it safe?
(2) Can she take the covidshield vaccine while taking defcort 3mg as it is a immunosuppressant?
(3) If yes, does she have to discontinue defcort few days prior (how many days prior) to the vaccine and post (how many days post) vaccination?
(4) Does covidshield vaccines have the live virus in them which can cause corona virus in the immunosuppresed patients?

Please let me know. Thank You

Brief Answer:
Get done CBC and CRP report. If both these are normal she can take vaccine.

Detailed Answer:
Hi,

Answer to your 1st question.

Yes, covishield vaccine is safe and can be taken by her. Get done fresh CBC and CRP reports. If both these are normal then no need to worry. She can take vaccine.

Answer to your 2nd question.

Yes, she can take vaccine while on defcort.

Answer to your 3rd question.

No dose modifications required.

Answer to your 4rth question.

Covishield is live attenuated vaccine. Means live Virus is there but not virulent.

Hope I have solved your query. I will be happy to help you further. Wish you good health.
Thanks.

Thank You for your prompt and clear response Doctor. I have a couple of follow up questions

(1) You mentioned that there is no need to stop defcort, will the vaccine still be effective if it is given while on a immunosuppressant?

(2) You said, "Means live Virus is there but not virulent". Can you please shed some light on this? is there any chance or risk of her catching the virus since the virus is already there in the vaccine?

Thank You

Brief Answer:
Yes, there is always risk of infection.

Detailed Answer:
Thanks for your follow up question on Ask a doctor forum.
I can understand your concern.
Both these are under study.
No one knows whether it will be same effective for those who are on steroids vs for those who are not one immunosuppressive drugs.
But better to take it as it will provide some protection if not full.
And we have seen patients getting Covid positive after 7 days of first dose of vaccine. So there is always a risk.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.
