By PPMS you meant Primary-progressive multiple sclerosis. Correct me if I am wrong.
Since exacerbations are more commonly related to the relapsing kind, but still an acute exacerbation can occur in your case , too.
Gabapentin and Diclofenac sodium should be taken for pain , if there is associated spasticity , baclofen may also be added. TCAs are also indicated in pain disorders in M.S.
If the pain persists or condition worsens the Steroids and Plasmapheresis are the next option to look and assess for. Doppler Ultrasound and x rays of hip and leg should also be done in consult with your doctor.
