Can Seborrheic Dermatitis Cause Severe Hair Loss?

Question: Hi Doctor,

I am having hair loss which started around 2.5-3 years back. My hair line has not yet receded but I can notice reduced density especially around temples. Still people can't notice that I have hair loss. I see lot of hair fall during shower/ shampoo. On my mother's side there is no history of hair loss. My father had male pattern baldness. I was born quite late, my father as around 55 when I was born so by then he had full baldness ( hair only on back and sides). Looking at old pictures I think his hair loss started in mid thirties. Apart from this I smoke around 1 cigarette a day. One more thing is I often notice yellow oliy sticky fluid coming out from my scalp which forms hard lump. This fluid I also notice around back of ears. This fluid is especially noticeable if I have not shampooed for 1-2 days. I also use minoxidil foam since last 1 year. I have attached my latest WET hair picture. These are my questions:

1. Is this seborrheic dermatitis. If yes how to cure. I have just ordered Ketonext Coal Tar and Ketoconazole Shampoo to use twice/thrice weekly

2. Is my hair loss due to seborrheic dermatitis. If yes then will it stop/regrow after this is resolved.

3. Is my hair loss due to male pattern baldness inherited from my father. If yes I want to know how fast it progresses. how many years I have before I have bald head.

4. What is the cure if possible.

5. Does smoking aggravate hair loss. I smoke only avg 1 cig a day. And I tried stopping smoking for 3-4 months but didn't notice
any significant difference so started again.

I have read your query.
I have seen the attached photograph also.
I understand your concern.
I will try to solve al your issues to my best.
As per details, it seems that you are having Seborrhoeic capitis et seborrhoeic dermatitis. Seborrhoeic dermatitis can lead to hair loss due to weakening of hair root as a result of scratching .
In such cases, I prefer to advise topical cream containing Clobetasone and miconazole to be applied over affected area.
Also anti dandruff shampoo ketoconazole based helps. Once you start using the medications, the condition responds to within 2-3weeks. I also advise my patients to use anti dandruff shampoo on alternative day basis initially.
In your case, it seems that you are having Male pattern Baldness also known as Androgenetic alopecia.
You have genetic predisposition also. It is the beginning of male pattern baldness. It is characterised by miniaturisation of hair which appears as reduced density and ultimately hairfall.
In your photograph, I can see the spars hair over crown.
If treatment for this is started timely then there are good chances of hair regrowth.
I prefer to advise topical minoxidil 5 percent and oral finasteride to my patients.
It will take atleast 3 months before you can appreciate hair growth.
Smoking is injurious to health and should be discouraged. It may contribute to hairfall to some extent.

In XXXXXXX , it is available with many brands like Eumosone M cream. You can use any ketoconazole based shampoo like Scalpe, AF shampoo, K 2 zole to be used on alternate day basis
You should visit your doctor to get the prescription. These medications should be used under doctor's supervision only.
