Can The Iron Deficiency Cause Low Bleeding During Period?

Question: Hi, I am 38 yrs old and during the last couple of years I noticed that about twice a year my period skipped for 1 month. Also, from 28-30 days, sometimes it started at 35-40. I consulted an endocrinologist and after blood test I got the diagnosis of Hashimoto, but my FSH and TSH where still in the average values, so she decided to prescribe Selenium, Vitamin D for my deficiency and a supplement based on Macca and Vitex Agnus. 1 monthe after started the treatment, my menstruation skipped for an entire month, then, the next month it started punctually with brown discharges for about a week, then it changed with very light pink flow, stopped for a few days and since 26.06.2024 never stopped (bright red with light medium flow). I visited a gynecologist who made a transvaginal ultrasound and told me that everything is OK except for a 3 cm cyst on one of the ovaries. Until now, I never had any problem during the ultrasounds or Pap test etc. She prescribed Duphaston for 10 days to stop bleeding (which didn't happen at this time) and said it could be the cyst or my hormonal imbalances seeing that my Estradiol is pretty low for my age. I have also mentioned that I suffered for iron deficiency many times and I do have some of the symptoms now too, but didn't believe it can be the cause. Also, after the first day of taking Duphaston, I started to feel a bad odour of the urine and a light-medium burning sensation while urinating. I would need an advice regarding this situation. What other tests should I do? Can the iron deficiency cause this bleeding?

Please test for urinalysis and urine culture and sensitivities

Hi dear
Your symptoms seem to be related with urinary tract infection for that we need urine test and it’s better if culture and sensitivity testing also done . As you have anti tpo antibodies in report which also weakens immune system in few people, and produces good chances of urinary tract infection. For 3 cm cyst gynaecologist medicine can be taken but follow up is very important to see it’s size status. Continue your thyroid meds as prescribed as your levels are now in control. Please send your CBC report or get tested for it ,so that we can relate your hemoglobin and period irregularity. In your case more likely due to hormonal and cyst related bleeding is cause but still if you have anemia it can produce periods irregularity too for that cbc have to be done . Best is follow up with your Ultrasound and hormones level with CBC and urinanalysis with gynaecologist.

Hello,

Thank you so much for the detailed answer. Meanwhile, even after 1 week of taking Duphaston (Dydrogesterone), as the gynecologist prescribed, the bleeding (even if not heavy) is still there.
But some of my blood tests are out, and I would like to know if there could be some indicators of the bleeding origin. Thank you

Best way is to get surgery for cyst for immediately relief of symptoms

Hi dear
Usually if cyst is 4cm or more surgery can be done until then medical treatment is given . If you have still symptoms with medication you can opt of cyst removal.send me your ultrasound results.You have low folic acid , is your gynaecologist given meds for that ? and have they prescribed you treatment for urinary tract infection? As your hormones reports also indicate possibility of premature menopause tendency but most likely cyst is causing hormones changes . Along with all above things also include few simple things which I saw worked for many females . Healthy natural diet ( more fruits and juices and antioxidants amla ,aloevera etc avoid non veg alcohol etc ). Do yoga half hour and learn sky breathing exercises. Stay away from stress
