Can The Lumbericals Be Built Up Again In The Hand With The Thenar And Hypothear Muscles After Ulnar Reinnervation?
Question: Hello Dr I need to know once I have ulnar reinnervation In my hand as I had an ulnar nerve entrapment and had a anterior interosseous nerve transfer with a ulnar transposition at the elbow. Can the lumbericals be built up again in the hand with the thenar and hypothear muscles? as it hurts to grip certain objects even If I use padded gloves. I try to do weight training but even with padding am I at risk damaging Myhand? Can those muscles of the hand be built again?
Brief Answer:
The muscles of hand can built again.
Detailed Answer:
Hello,
The lumbricals are small muscles and proper physiotherapy can help in regeneration of the muscles. The muscle bulk can be built up again. Along with thenar and hypothenar the lumbricals will also gradually built up.
The mild pain even on wearing padded gloves is due to stress and if the pain is a mild one that stops after giving rest to the hand then there is no need to worry.
I hope this answers your query.
In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out.
Wishing you good health.

Hello Dr So as long as there is reinnervation by 18 month's which I'm sure there will be as I am now two months post operation and I have only slight improvement in my hand intristic muscles basically I can manage shoelaces now but up to a month ago I couldn't. So the hand muscles lumbericals and the thenar and hypothear muscles can be all built again? The question with holding a bar only while I hold it with a pad there is some pain but none after I'm scared does that damage the median nerve and ulnar nerve which isn't working in the palm of my hand?
Brief Answer:
Exercise will not damage ulnar or median nerve.
Detailed Answer:
Hello,
The building up of muscles is a slow process and it depends on the extent of exercise the muscles are getting.
Holding a bar can cause pain due to lack of muscle mass. But this does not affect the ulnar or median nerve. Continue with the exercises as long as there is not much pain.
Hope my answer is helpful.

The holding of a bar I use a sponge pad to ellivate alot of pressure but still is pain so this won't damage the hand more? No there is no pain after. I plan to do every excersise possible for the hand so that all the muscles can be built again. I take all the supplements of B1 B3 B6 B 12 Vitamin E Vitamin D2 Alpha lipoic acid and do laser therapy with LED lighting as apparently that helps with nerve regeneration. What will I feel in my hand when the ulnar nerve signal starts to reach it?
Brief Answer:
Nerve signals can cause tingling.
Detailed Answer:
Using a sponge pad will decrease the discomfort. When the nerve signals will start reaching the hand you can feel tingling or current like feeling in the area. This can be without any symptoms also.

So the pads are safe to use as long as there is no pain after wards In my hands? Of there is no tingling is there any other way of knowing that the signal is there in my hand as sometimes I have burning sensations in my fourth and fifth fingers but I know the ulnar nerve signal has not reached the hand yet
Brief Answer:
Nerve conduction studies can help.
Detailed Answer:
The pads are safe for use.
Other than tingling the nerve conduction studies can help in finding out if the signal has reached the hand or not. Taking methylcobalamin supplements can help in decreasing the burning sensation that you have.
