1. The commonest cause for stroke is either bleeding or clots in the brain. This can happen when the blood pressure shoots up suddenly and or if the patient is on any thrombolytic or anti-coagulant agents (warfarin). The increased blood pressure can also cause tearing in the wall of the aorta.
2. Sinus infection cannot cause neck pain. The neck pain was because of the tearing of the aorta. Taking decongestants cannot cause stroke.
3. Headache could be because of sinus infection, raised blood pressure and even stroke.

Again Dr. it was a severe case of sepis phen, UNTREATED lungs, sinus, ect.
So Warfarin "would or Wouldn't be a drug prescribed? In your expert opinion after what I had explained to you, would you prescribe warfarin 5 to this patient and also can the art repair the tear itself in time??
Stroke is not a direct consequence of pneumonia...

Thanks for writing back
I would like to bring to your kind notice that untreated pneumonia and sinus infections do not cause stroke. In fact, the untreated sinus and lung infections will increase the blood pressure because of the stress undergone while breathing strenuously. The increased blood pressure along with warfarin would have caused bleeding inside the brain and thus the stroke.
Secondly, the pneumonia can mask the onset of stroke because of the respiratory embarrassment. So the stroke would have taken place a few days back and the affected individual as well as the doctor would not have realized it. Hence, I would like to stress again, the stroke is not a direct consequence of untreated respiratory infections.

Welcome back
1. I'm sorry for the misinterpretation regarding the use of warfarin. Apart from that, I would adhere to the response I gave earlier.
2. Warfarin is recommended in those cases which are expected to have blood clots (such as stroke), arterial thrombosis, major surgery on the heart and lungs, myocardial infarction, etc.
3. This is a tricky situation wherein he would require warfarin, if the stroke is because of clots and at the same time there is dissection of aorta, which can be detrimental. But, in this situation, I would still prefer to use warfarin while regulating the blood pressure.
4. The management of the aortic dissection depends on the location of the tear. If it is in the arch, the repair has to be done immediately. And, if it's located elsewhere, it can decided once the blood pressure is under control. Small tears can either be left alone or a stent placed.
5. CAT can is not the investigation of choice for dissection of aorta. Angiography has to be done to look for the weak point. And, MRI Scan to map the areas of damage.
6. The cause of the confusion is the reduced blood supply and swelling in the damaged area of the brain, as a result of stroke. Hence, warfarin would be ideal in treating this.
