Question: Hello Dr.
I am worrying about a case that happen just few days Ago , I have asked for massage service. The massage lady have arrived and We went to shower first and she washed my body and did soft hand job for me using lotion and soap. Then later on during the massage i was wearing my underwear and she was fully naked. I have actually sucked her chest and nipple heavily but I did not notice that if she is actually have breast feeding milk. I got panked when I asked her about her actual job as she is sex worker and have seen two customers before me on the same day. Also I have noticed that she had like healed skin rash on her right shoulder when I asked she said it%E2%80%99s not an allergy but she had this since born only on this part. It was on the side of the shoulder.

1- Can you please advise if such experience may transmit HIV ? What is the chance of infection ?
2- what about other STD%E2%80%99s ?
3- do I need any testing for STD%E2%80%99s ?
4- can the STD Iike HIV transmit via hands cuts or from her hand when she touched my genitals? I have not seen blood but just in case if there were micro cracks ?
5- some doctor said I have to wait for two weeks and get tested so I can be 100%25 sure which even scared me more.
6- started to have headache and night sweating a lot and not sure if this has anything to do with this incident or because of me worrying so much.

Brief Answer:
Your HIV/STI risk is negligible

Detailed Answer:
Hi,

1. Hand job and breast sucking are the safest sexual activities as far as HIV/STI transmission is concerned. If she was lactating, then you would have identify the same during sucking heavily.
2. Other possibilities of STIs are unlikely
3. No test is necessary as long as your activities were as explained by you.
4. This possibility is negligible.
5. Guessing the chances and assessing with symptoms are not at all reliable as the symptoms are vague and can occur in many other situations. Ruling out or confirm the infections with appropriate tests are only way to confirm.
6. These symptoms can occur to anyone in the abscence of HIV/STI and also cane be symptoms for anxiety also.

I Need two clarifications from your side:
1. In the patient's profile the gender is mentionesf as female... Hand Job?
2. Adress mentioned as Saudi Arabia... Massage parlor in Saudi Arabia? That too, she was naked?

Brief Answer:
Your HIV risk is negligible

Detailed Answer:
Hi,
Welcome back.
HIV cannot be viable outside the body fluids not more than few seconds. HIV transmission needs exchange of body fluids.
Theoretical possibilities are there. But transmission through casual contact is not common.
I mentioned that your risk is negligible and not nil. (https://www.cdc.gov/hiv/risk/estimates/riskbehaviors html#:~:text=*%20Factors%20that%20may%20increase%20the,%2C%20and%20pre%2Dexposure%20prophylaxis.)
I hope that you got the point.
