Can you please check the attached report and help me with the proper plan of action to overcome these issues I have repeatedly had discrepancies in my LFT and Lipid profile tests. There are some changes with the Urine this time as well.

This seems like a viral hepatitis. Kidney may also be affected. You need to drink lots of clean water and avoid any triggers like alcohol, medicine, supplement or any chemicals that may affect liver or kidneys. Get a whole abdomen ultrasound scan to check for any liver or kidney issues.
You may start taking following ayurvedic medicine:
Syrup Liv 52 1 tsp twice a day for one week. Take rest. Consume food that is easily digestible.
Most virals are self-limiting and hopefully this will also get better by itself. Currently it is mild. If the yellowness worsens or any further issues in the reports, please see a doctor.
