Question: I have the problem of heartburn/GERD.
Couple of years I was diagnosed with Gastritis and I was on esomeprazole for 3 months.
Right now I am not taking any medicine for GERD . But still my stomach is sensitive after years.
I avoid citrus fruits ,coffee and spicy foods.
Recently I have started to take a medicine called nortriptyline 25 mg which is giving me heart burn and pressure feeling in throat. I am reluctant to take esomeprazole as it gives me gas with my previous experience. The same is the case with pantoprazole.

1) why PPI gives me gas.

2) Can you suggest a solution so that PPI does not cause me gas

I also want to bring out another history of mine 4 years back when I moved to a new city. As a result of restaurant food, I got food poisoning many times during a span of 1 year and it went away after antibiotic treatment each time. By the time I decided to eat only home cooked meals there was something which was not right with the bowel movement. The stools are not well formed. It lacks the sausage shaped rigid consistency. But apart from that I didn't face any problem after eating home cooked meals. I still have issue with stool even though I am taking home cooked meals from last 3 years. Also I don't have much tolerance towards outside food even now. After checking with practitioners , I got two issues which may be causing this.
1) Your gut flora has become weak. For this I am taking isphagula husk power 5g morning with one probiotic capsule.
2) There is a functional problem with bowel movement which is causing food to move at a faster rate.
For this issue my psychiatrist has recommended treatment with an antidepressant called Mirtazapine.
what do you think about this medication . I believe this medicine needs to be taken only for a particular time period after which we can taper and stop.

3) Is there a different medication for correcting the bowel movement .

Thanks for query,
Firstly Mirtazapine is given to sooth your mind.
Very likely the doctor is thinking about IBS.
When nothing is found wrong with patients in test but they still have symptoms like you are having, Modern medicine terms it as IBS(irritable bowel syndrome).

Now autonomous part of nervous system works automatically so person with anxiety stress (may be subconscious could have stimulated vagal nerve).
This hyper stimulated vagal nerve can be a cause of symptoms as probiotics and isabghul husk have not helped you.
Now let's move to esomeprazole.

Very likely it's a coincidence that you were consuming large amount of nonveg food while taking it.
Non veg food is not easy to digest..is heavy and sometimes fermentation inside intestines cause release of large amount of toxic gases.
But let's take that Esomeprazole was causing problem.
In that case you could shift to H2 blocker Ranitidine 300 mg once a day or 150 mg twice daily and note the change.
Diagnosis,
Suspected irritable bowel syndrome(as no test can diagnose it).
Treatment-
Mirtazapine 7.5 mg once daily initially and note the response.
Along with it a good quality probiotic like VSl#3 two times a day.
Avoid isabghul as it is used for constipation treatment.
Take Ranitidine 300 mg once daily in place of Esomeprazole.

If possible try to detoxify your body by taking vegetarian food for 10 days(eat lots of fruits like apple pear banana kiwi).
