Could A Colonoscopy Actually Get Into The Cecum?
Question: I have been reviewing a colonoscopy report from 2010. It was a clean report. The is a statement in the report related to the CEcum. The report said the scope aa passed through the colon under direct vision to the cecum.. The report then said---''The cecum was identified by transillumination of the light of the endoscope on the RLQ ,visulation of the ileocecal valve, the appendiceal orifice and the triangular fold….=== My question is did the scope actually get into the Cecum? If it did and there was direct visual images of the inside of the Cecum, why does it say that the cecum was identified by translumination, etc if the camera had direct vision to the cecum?

The endoscope was inside the cecum only. Only because it was inside the cecum, the endoscopist was able to see the ileocecal valve, appendicular orifice and triangular folds. These three structures are inside cecum and the endoscopist was able to see them with the aid of colonoscope.
Transillumination is just another way of confirming that colonoscope has reached cecum

Thank you for the explanation. I am wondering why the report included the additional information about the transillumination. Is this just to prove the scope was in the Cecum? Its confusing because if the cecum was seen by direct vision. Why would a statement be included that says the "Cecum was IDENTIFIED by transillumination". Should it just say and verified instead of identified by transillumination? XXXXXXX

Sometimes if the procedure takes a longer time and the colonoscopy picture is not clear we look for the transillumination to know whether the colonoscope has reached right lower abdomen and then we manipulate the colonoscope and reach the Cecum and identify the structures in cecum.
But whatever may be the case, there is no need to mention about the transillumination in the report as it is not going to convey anything.

wow, what a great explanation. I am still puzzeled as to why they used the term- ' the cecum was identified by the transilluminaion -etc''''' . I guess it was not written wee and should not have said this. Instead, it should have used terms to say verified by transilluminatio. but thanks , hcm drs are great.
