Question: Could benzyl alcohol cause a delayed allergic reaction that could cause respiratory issues?

Is it ever used as a preservative in allergenic extracts used in scratch and intracutaneous testing?

Benzyl Alcohol can not cause delayed allergic reaction. It can cause immediate skin reactions like contact dermatitis, erythema, rashes, Itching etc at application site.
Moreover it can not cause respiratory issues.
So I don't it is a cause for your respiratory issues.

I am talking about when it is used as a preservative in IV medications. I know it can effect neonates and in high doses it can be toxic.
