Brief Answer:
It could be due to infection etc.

Detailed Answer:

Hello,

There seems to be some complication after the D and C. First periods after D and C are anyways heavier and associated with pain and discomfort. Severe back pain may mean complication like infection, retained products of conception etc.

You need to get back to your treating doctor. She may do some tests like PAP smear and pelvic ultrasound. According to the investigations and diagnosis, further management can be done.

Hope this helps.
