Based on your query, my opinion is as follows:
1. If you have cardiac symptoms since childhood, after being affected from scarlet fever and also prone for infections from that period, then it could be the cause.
2. However a-fib is due to electrophysiological changes and is commonly secondary to infarction, fibrosis or electrolyte imbalance.
3. Infections and weakness can be explained due to reduced nutrition and reduced immunity. Associated anemia or hypothyroidism requires evaluation.
If present since childhood, yes, scarlet fever can explain this. For recent last few years onset, need to look for other causes as suggested above.
