Brief Answer:
I need to see an attached picture of this lump .

Detailed Answer:
Hello,
I have reviewed your query and here is my advice.
Related your symptoms I need to see an attached picture of this lump and I suggest to make an ultrasound of the left testicle in order to define better the diagnosis.
As well I need to know if you have done recentlly unprotected sexual intercourse and if you have urethral discharge during this time.

Hope I have answered your query.
Feel free to ask me further.

Brief Answer:
The lesion near the lump looks like a genital wart.

Detailed Answer:
Hello and welcome in Ask a Doctor.

I have seen carefully your picture and I think that this could be related with an infection.
But in this situation is not easy to say that this lump is related with STI just with a picture. It needed a physical examination and some lab examination (chlamydia, gonorrhea, syphilis) to define better the diagnosis.
But I need to ask you related to the lesion near the lump because it looks like a genital wart. How long you have this lesion?

Hope I have answered your query.
Feel free to ask me further.
Regards.

Brief Answer:
Please don't worry!

Detailed Answer:
Hello and welcome to Ask a Doctor.
I am sorry for my late response.

Yes, usually genital warts are more spread out.
So, if you have this spot for a long time without multiplying exclude the option of genital warts.

Hope I have answered your query.
Feel free to ask further.
