Could You Please Assess My Annual Test Report?
Could you please assess my annual test reoprt? It shows my Egfr level is 2, even last year's report shows that it was 2.
could this be concern? if so, what other tests should I get done?
I lead a healthy life, into wieght trainings and take 3 to 4 grams of creatine supplement 4 to 5 days a week.
It doesnot seem a kidney issue
Detailed Answer:
Hello,
I have seen your report and here is my opinion.
eGFR is calculated as per following equation:
eGFR (mL/min)= [(140 - age) × Wt / (0.814 × S.Cr in µmol/L)] × (0.85 if female)
As per this equation, if the Sr Creatinine is high, eGFR will be calculated as low. Since you have been taking creatine supplements, it can cause an increase in Sr Creatinine levels.
Since when are you taking these supplements?
Since your kidney function test and urine analysis are normal, I donot see any reason for invasive tests like kidney biopsy. However you may get an ultrasound of kidney, ureter and bladder done, just to rule out any kidney issues.
I think you should stop creatine supplements. Also you may repeat the kidney function test one week after stopping the creatine supplements.
I can also see that you liver function test is abnormal. Do you have jaundice? Are you taking any medicines or alcohol? Do you have fever?
Hope this helps. Let me know if I can assist you further.
If you still want a nephrologist to answer this query, you can post another query for him.

My Bilrubin level are aways slightly high as this is due to Dubin-Johnson syndrome, i don't have any fever or health issues.
I am physically very active and into weight training.
Your kidneys seem healthy
Detailed Answer:
Hi again
Thanks for reverting back with details.
Dubin Syndrome explains raised bilirubin levels.
If you can tell since when you are taking creatine powder, then there could be temporal relationship with increased sr creatinine levels.

I would have started it last year around June/July. Since its the most studied and researched supplement and even doctor had recommended me it too as I am a vegetarian long time ago.
At the age of 23 /25 i used to take it and then may be at early 30s I started it again for a few months.
But I was also told that if you are creatine supplement, it will show elevated creatinine levels in your reports.
You can stop creatine intake
Detailed Answer:
Hi ,
It's highly likely that sr creatinine is raised due to creatine intake.
One method to confirm correlation: you can stop taking creatine and check sr creatinine levels after 3-4 weeks. If the levels are decreased, then the increase was due to creatine intake only
