I have examined the MRI images that you provided.
As you may understand, the evaluation is limited because all MRI series should be evaluated.
In the provided images I can see a thicker (probably swelling) of the right optic nerve, mostly linked with inflammation.

Thank you for reviewing what were a limited series of images for the MRI. We have received the report from the MRI scan and I have attached it.
Based on the findings in the report, in your opinion what would be the best way forward in terms of diagnosis? Is a PET scan indicated? Can a lumbar puncture still provide more diagnostic value? Are there further blood tests that could be recommended?
We are still awaiting the Immunoglobulin IgG subclasses test to come back. HIV and syphilis tests are negative. Cat scratch serology tests are negative with B henselae IgG below 64 and B henselae IgM negative. As we are in regular contact with a cat, are there other forms of bacteria that could be transmitted by a cat particularly through saliva that could be picked up on a blood test?
Would appreciate your opinion.

I would like to follow up on my query below. I am wondering in terms of sub-specialisation which neurologist would be best placed to advise on the condition as it appears on the scan. We have been advised to follow up also with neuro-muscular and neuro-physiology specialists so as not to restrict to just the neuro-immunology area. Does the scan suggest that neuro immunology is the best area to focus on? Or is there another sub specialist in neurology who should be consulted?

Lumbar puncture and cerebrospinal fluid examination may also give some information regarding autoimmune disorders and any infection.
More often, cats can transmit parasites, but rarely bacteria.
Besides neuroimmunology, an infectious diseases specialist may be of value in this case and may order the necessary blood tests if the infection is included in the differential diagnosis.
