Question: Dear Doctor,

I am 31 year old married man.
I was regularly going to the gym till August -2019. But after I became irregular due to hectic schedule. I started gym with light workout again in the end of January -2019. But I could not work out. During strength training I felt stressed and when the workout was over I felt like I had low energy. Although, I could easily perform cardio workout. After meals I felt better. But later lockdown started and I missed gym for a month. I started workout again a few days ago at home. Three days ago I started with 2 sets of 20 pushups, total of 40. 40-50 squats and 10 mins trademill and 10 mins cycle. After warmup I did 20 push ups and then 3 mins rest then again 20 push ups. After that my heart rate increased till 130bpm and later I felt one skipped heart beat. I took 3-4 mins rest and then did trademill for 10 minutes and then afterwards I again did 40 squats and again after that I felt a skipped heartbeat. After that I stopped exercise since I was worried but then I felt fine. Today when I was typing this mail I became anxious and I felt 4-5 skipped heartbeat so I stopped typing and later I felt relaxed.
A similar episode occurred with me in XXXXXXX - 2017 when my uncle passed away, I was stressed after watching all that and those skipped beats happed to me again. I was scared and went to the doctor, he took an ECG and found ectopics. He said there is nothing to worry at all and they happen due to anxiety.
I don’t get these skipped heartbeats so often but they are scary. In November 2019 I did following tests since I had taken a term plan from the bank and all came out normal.
1.     Blood pressure
2.     CBC
3.     Blood sugar
4.     Blood urea, creatine
5.     HbA1c
6.     ECG
7.     Blood Nicotine test
8.     Hepatitis and HIV test

All the test reports were normal. However I checked Vitamin N12 and Vitamin D and I found they were deficient and I taking medication for the same.

Whenever I am anxious or terrified I get these skipped heart beats.

My question to you is am I pushing myself too hard during workout? Are these Skipped beats serious since I am just 31 years old. What should I do to avoid them? Do I need medical evaluation?

Hello,
Skipped beats or ectopics occur quite commonly in young individuals and most of those are benign and not worrisome. Evaluation is done with echo just to rule out the possibility of any structural heart disease.
As you don't any symptoms like chest pain, shortness of breath or dizziness/ syncope, its unlikely to be a major issue and these are probably a benign ectopics. Also, if any heart disease was there then it would have manifested by now as you are having those although intermittently from 2017.
So I don't think there is anything to worry about it, however echo is recommended in all ectopics patient just to be sure especially if you didn't have one in the past.
You may do workouts, but its always better to gradually escalate the exercise regimen over few days or weeks rather than being abrupt. Stress, anxiety, sleep disturbances, gastric disturbances, caffeinated drinks or too much coffee, alcohol, smoking tend to aggravate these, so try to cut those if you are having any of those. You may perform deep breathing exercises or yoga to calm your anxiety.
If you are too symptomatic then you may get medicine prescribed like Propranolol to suppress those otherwise not absolutely necessary if you can tolerate those.
So overall, usually not worrisome, but echo is recommended just to be sure.

When I take deep breaths, they go away soon. Is it okay to have those kinds of beats and why do they happen? I started gradual work out today as you said with 10 pushups and 10 squats, but I couldn’t perform. I felt breathless but when I sat down, I felt better. Was it because anxiety or something is wrong with my heart? Since last 10-12 days this was the worst performance during workout.

I do not smoke, I rarely have coffee, I rarely have alcohol in less than moderate quantities, but I do become anxious soon, I have stomach acidity issues and feel bloated and a little breathless after meals some time, but that is no so bothersome. What could be the reason behind skipped heartbeats? Will they keep happening for the rest of my life? Truly speaking, I see a significant component of anxiety in your symptoms. Your symptoms are classical of ventricular ectopics, after each ectopic there occurs a pause and strong beat subsequently. Its usual for ectopics.
So these ectopics have triggered fear and anxiety in you, which is probably causing lot of symptoms. Due to this, your awareness about your beat has increased. Normally after walking stairs or any physical activity, normally heart beats faster and stronger which usually goes unnoticed due to less awareness, which is now increased you. These usually come in episodes and these episodes may become less intense and may completely disappear after few years or decades.
There is no identifiable cause in most of individuals and it's due to some derangements during heart formation which manifests as ectopics later in life. However, in rare cases, it's due to structural heart disease so echo is recommended to be sure. So, get one echo done just be sure and continue to have workouts as much as possible and gradually increase them. Your performance is affected probably due to your anxiety and panic.

I forgot to mention that day when I had 4-5 skipped heartbeat they came in 2-3 minutes.
I have started work out gradually as you said with 10 pushups in one set and 4 such sets total 40. And 20-20 a total of 40 squats 10 mins trademill and 10 mins cycle. I did that yesterday and today. In the initial sets I became breathless for 2-3 minutes after pushups. Then in the consecutive sets I became breathless for less time and I required lesser rest. But I could not do 20 in a row as I used to do few months ago. however, Squats are easy for me and I can do 30 in a row. I was anxious today, so I worked out with a little difficulty. When I’m nervous I get this feeling of having a cardiac arrest or some major emergency and I start to panic. Today I was tired and when I climbed the stairs carrying my 10 month old daughter, weighing 9kg, I felt breathless for 3-4 minutes and better after that. Is it ok? I there nothing wrong with me?
My question to you is if you say skipped heart beats are nothing to worry then why do they occur to me and not everyone? Since the feeling of having one is dreadful and very scary.
Can they happen without me noticing them? Will they damage my heart in the long run? What should I do to so that I stop experiencing them.

Your exercise intolerance is probably due to deconditioning over the months coupled with your anxiety. You may continue your work outs and gradually escalate them. It's normal for individuals to experience breathlessness on climbing stairs with 9 kg weight.
These may go away with time or else you may have beta blockers like Propranolol to stop those. Echo should be done to be sure if not done recently. Overall, I think these are benign ectopics.

As I mentioned earlier, we should rule out the possibility of structural heart disease with an echo. So you ll need a visit for an echo atleast.
But if we think in terms of probabilities, this is unlikely to be a worrisome issue and we need to be sure on echo. However, considering your symptoms, history, I think this is benign only, but I don't want you to completely rely on this as we may miss certain things online.
In my opinion, this would subside with time and wity relaxation techniques. There is a strong component of anxiety in your symptoms, and you need to take focus away from it.
Beta blockers need not be taken lifelong. They would be tapered off after few weeks once ectopics are stopped.
So overall, I think these are benign and not worrisome and should settle, but I want you to be sure with personal doctor visit and echo as well. All the best.
