Question: Dear Doctor,
I am 36 year old , Weight 94 kg
height 5ft.5in. Obese
G6PD deficient.

I am having high blood pressure problem and I am having anxiety issues in the past.
For Anxiety I was taking Pexcep cr 12.5 and loanzep 0.25 and later after 3 years I stopped as it felt better.

Currently I am taking telmikind 40 in morning at around 11:00 a.m. and nebicard 5 in the evening at around 6:00 p.m.
My problem is that I I get very tensed and my pulse increases when it comes to my health and maybe I have white coat hypertension issue that is my blood pressure comes High when I visit doctors office or when I am tensed about my health.
It was all fine until Recently when I check my mother in law blood pressure I got tensed when I check my own pressure at home. Since then I feel head heavy and my Currently my pressure is showing 175 /100.

My doctor advice me to take telmikind 40 twice a day with 12 hour gap along with nebicard 5 in evening.
He told me to relax and stop worrying about high BP and anxiety.

He also told me that if reduced by 10 to 15 kg is my blood pressure will be reduced significantly.

When I said that may be due to stress or anxiety my blood pressure must be increasing he told me that white coat hypertension will increase the systolic blood pressure and not the diastolic pressure.
Since my diastolic pressure is close to 100 you have to take blood pressure medicines..

So can this be because of anxiety? Or should I again start anxiety medication.
And is my current medication good enough.

Thanks
Regards

Brief Answer:
You need combination therapy

Detailed Answer:
Hello

After going through the medical details provided by you I can understand your concern for your health and I would like to tell you that you should follow the advice of your doctor and take the medication has advised because your blood pressure is quite high for your age.

Also I would like to suggest you to kindly take the help of a psychiatrist to help calm down your anxiety levels.

Also discuss with your doctor about getting a renal artery Doppler scan done to rule out any stenosis which is an important cause of hypertension in young individuals.

Kind Regards

Dear Doctor,

As you suggested renal artery Doppler scan. I understand that it is related to blood flow in kidneys. I would like to state that I had recently done normal sonography of abdomen and that time my kidneys were normal as per reports. So is it ok.
Also does anxiety increases blood pressure??
It increases systolic or diastolic or both??
Thanks.

Brief Answer:
No need for repeat scan then

Detailed Answer:
Hello,

If your previous ultrasound was normal and was not suggestive of any sort of renal artery stenosis and there is no need to repeat the scan so early.

Yes, anxiety is a very common and manageable cause of high blood pressure although it usually affects systolic BP in some individuals, both systolic or diastolic and in some only diastolic is affected.

Kind Regards

Dear Doctor,

First of all thank you so much for your advice.
My only query is now that I am taking Tazloc CT 40 in morning at 11.30 after breakfast and Nebicard 5 in evening 6.30. I do get a relief after I take those medicine but not completely.
My doc says it will take time to adjust but I get irritated when my head gets heavy.
He also told me that I can take nebicard 5 in morning with Tazloc CT 40 and Nebicard 5 in evening 6.30pm.

When I asked that if I take nebicard 5 two times in day with Tazloc CT 40 will it be a high dose??? He told me Nebicard is mild.

For stress i am taking Etibliz 0.5 half in afternoon and 1 at night.

Thanks

Brief Answer:
Hello again

Detailed Answer:
I would like to suggest that as you are quite young so first try reducing your weight by following healthy lifestyle and daily exercise as that will also help reduce your anxiety levels.
So continue with your same medication first. For anxiety keep taking what you are already taking.

Kind Regards
