Question: Dear Doctor

I know that giving breast milk to a baby is best.

However is it ok to add a scoop of formula powdered milk to breast milk instead of water and give to the baby as it might be more nutritious ?

You should not be adding water to breat milk at all

Detailed Answer:
Hello... I understand your concern.
But you should not be using water to breast milk at all.
Please feed undiluted breast milk exclusively till 6 months of age.
Breast milk is baby specific and is 100% nutritious.
DO NOT ADD FORMULA TO BREAST MILK PLEASE.
