Question: Dear Doctor as I told you I have the problem of stasis Eczema becoz of varicose veins on both legs leading to black spots, itching sensation, dry skin and ulcers. My request is I will get it done doppler test after lockdown. In the meantime kindly advise what medication to take, ointments to apply and compressed stock is required etc. I sent photos to your email. Waiting for the reply please.

Hello and welcome to Ask A Doctor services.
I have read your query and evaluated attached photo images of both legs showing unhealed patches of skin and here is my advise.

* As per my surgical experience, the lesion shown is most likely due to
stasis dermatitis over eczema portion which relate to obesity and
sedentary life style induced sluggish peripheral circulation in most of the
cases.

* Varicose veins may or may not be a counterpart of this condition and
hence you should not delay your therapy for waiting of doppler report.

My essential guidelines are as below

- Keep the legs elevated over 3 pillows when resting.
- Do not spend much time in legs dependent position.
- Keep blood sugar, body weight under control, as mobilisation is key to keep
blood circulating precisely in lower limb particularly.

- So far as compressed stocking is concerned, I would not recommend it at
this stage before evaluation of artery and vein status.
- Local application of steroid cream as Betnovate N containing
Betamethasone and Neomycin cream as combination helping inflammation
control in nice way.
- Itching control can be obtained with cetrizine or diphenhydramine tablet.

Would like to discuss your further doubts in following session.

Dear Dr. Thank you for nice reply. Kindly tell me dosage of ointment and tablet also tell me whether ointment apply to affected area.

Hi
Welcome for the follow up.

* The ointment is to be applied as thin paste over the skin lesion three times
a day after cleaning with antiseptic soap as dettol or savlon.

* The cetrizine ( 10 mg ) is to be taken only when there is much itching, not
on regular basis.

Rest of the medications as oral antibiotic tablets, steroids or others are not recommended without examination.

Additionally I would like to add that if you are smoking or using tobacco in any form, you should strictly stop it from right now as it would be worsening the issue to unbelievable extent.

Dear Doctor, white patches are forming in between my both legs feet fingers as my fingers are very close I think this might be one of the reasons

Welcome for the follow up.

* The white patches formed between feet toes are most likely related to
fungal infection with Tenia.

* Local application of anti fungal cream as Miconazole ( 2 % ) for 7-10 days
and
prophylactic anti fungal medicine as Fluconazole ( 200 mg ) once at night
for 2 days will help to solve the issue.
