Dear Doctor, Good Evening. From Past Two Years My Dad

Question: Dear Doctor, good evening. from past two years my dad was feeling guiddiness and unconscious then and there when he thinks about something. it went on increasing day by day we didn't realise. we showed to general physician which they confirmed everything is normal like BP,sugar and all. Now we went to neurosurgeon and he suggested to take some test and confirmed he has a form of epilepsy. I have attached the reports and prescription. please give your second opinion on this. thanks regards XXXXXXX

As per reports, your dad has epilepsy (seizure disorder). It is arising from left side of brain (temporal region) due to a disease called as mesial temporal sclerosis (MTS), which was confirmed on the MRI brain. EEG also has shown epilepsy arising from left side of brain.
The drug chosen (Zen retard) is a good drug for this type of seizure. The dose may need adjustment depending on the response to treatment.

thank you so much doc, actually he is a active person. his age is 73. he use to get the problem once in a while whenever he thinks more. after having this drug he is feeling too drowsy and he couldn't even walk more. most of the time he sleeps. also he doesn't get this daily. whenever he gets this problem this happens two to three times. kindly advice, any serious issue? thanks

XXXXXXX retard can cause drowsiness in some patients. However, it improves after one week of starting medicine.
If drowsiness persists after that, then check SODIUM level in blood, as XXXXXXX retard can cause sodium to become low in blood (hyponatremia), which can cause drowsiness. Also check drug level in the blood to determine if the dose is appropriate by doing blood test (Serum carbamazepine assay).
If both the above are normal and your dad is still drowsy, we need to change the medicine. Options include valproate, lacosamide or levetiracetam.
