It's right bundle branch block with sinus rhythm, left axis deviation and left anterior fascicular block. No significant St t abnormalities seen.
This is usually an age or hypertension related change however possibility of ischemic heart disease should be ruled out with further evaluation and echo. Also, needs lipid and weight control.
So, patient should undergo echo and if normal then stress test like stress thallium/ treadmill test or CT coronary angiography to rule out possibility of ischemic heart disease.
