Question: Dear Dr
There is redness & irritation and little itching on the penile shaft under the foreskin near to XXXXXXX side for the last 3 days. There is no discharge from penies and no burning sensation when urinating. Took cetrizen tablet and applyed Framycetin skin cream. there is not much effect, but it is not spreading to other areas. Photo of the infected area is attached. I am taking medicine for diabetic for the last 3 years and also BP medicine. Sugar & BP are under control. My wife also a diabetic patient for last 20 yrs. She took treatment for yeast infection 3 months before. Dr. gave her clotrimazole vaginal gel for treatment. She is OK after that. There is no chance of infection from her as I have not done peneteration sex for these 3 months period till date. It is difficult me to contact a dermologist locally due to corona situation prevailing. Kindly advice for treatment, medicines and cream to be used for clearing of the infection.

I have gone through your query and reviewed the attached photographs.
It seems to be yeasts infection.
I would recommend you to apply topical antifungal cream like sertaconazole cream ( onabet cream) on the affected area twice daily for 2 weeks and take antihistaminic like tablet levocetrizine 5 mg once daily for 10 days,
Also take an oral antifungal like capsule itraconazole ( capsule onitraz) 100 mg twice daily for 5 days.
Keep the area dry and clean,
Get your fasting blood sugar, HbA1c and routine urinalysis to find out the cause of this infection.

Thanks very much for quick response. I have following few queries. Kindly reply:
1. Can I apply Clotrimazole 1% anti-fungal cream in place of Sertaconazole cream, which is now available with me (earlier used by my wife) ?
2. Any need of applying antibiotic cream in additional to anti-fungal cream as it appears that there is a very small cut in the infected area ? One week ago, by chance, my penies got rubbed with trimmed pubic hair of wife causing little pain in the shaft area at that time, which I think may have caused very small cut there.
Kindly advice further.
