Question: Dear Dr . XXXXXXX ,
I am 49 male hypertension controlled by medicine (co-diovan /concor/phyiotense)

Before 3 weeks I started to have high frequency of urination , with high quantity at each time I go .
(Almost .5 litre every 2-3 hours ) . So I did some blood tests , including kidney functions , liver functions , Galocouse , SPA for the prostate . Every thing was with in the limit . I did an abdominal ultrasound and no abnormalities were observed . Just then the frequency of urination has gone down and also the quantity and I thought I am ok again and blamed this problem for the diuretic in the blood pressure medication . . Then just few days after that the real problem started .and that what I want your opinion in (I thought I share the above history , it might have something to do with what follows ) :

I have been having the following symptoms for the past 10 days laterally :

- Faciial flushing (face , hands , feel hot , while my temperature is 36.8 c... feels like fever )
- Headache both sides of the head above the ears .
- Back neck pain left side .
-lLower back pain left side .
-Loss of appetite .
-Soft stool .
-uncomfortable stomach .
-numbness on hand (for just seconds when I wake up morning )

So I decided to visit an internal medicine doctor . After inspection , he said that the neck pain is due to spams and the back pain is due to some joint between the hip and the leg . He just gave me some muscle relaxer and pain killers . I did some more blood test and red blood cell , hemoglobin , ferritin were all high . I d share the result with you for your preview . 10 days later , I am having almost the same symptoms . Made we wonder maybe his diagnose is wrong . For the flushing and feeling hot body , he advised me to donate blood . Neck pain is better than before . Headache is less . But still some is there .But the lower back left side pain is just as it is . I am worried that this pain at the back might something more serious than the joint of the hip .
I don't know what to do or whom to see ..The internet is full of information and all symptoms seems to be similar . The high ferretin maybe infection ,but nobody seems to know . I kindly request your opinion in this regards . Thanking you in advance .

Hello,
I have reviewed your note and test results. Some of your problems may be stemming from your very low vitamin D. In addition, your glucose is high. Not high enough to diagnose you with diabetes but you may have a prediabetes condition. I suggest that you start on a vitamin D supplement and get a hemoglobin A1c test (it reflects your glucose control over the past 3 months).
Also I have a few questions.
Do you smoke?
Are you taking any supplements? and if so which ones?
Do you drink any alcohol? and if so how much?
Have you ever seen a gastroenterologist?
Please answer these questions and I will give you more help

Thank you so much for your reply .
1-Yes I smoke and I am overweight .
2- No supplements taken .
3-No alcohols
4-The doctor I have been seeing is an internal medicine doctor , I guss gastroenterologist is part of his job .

I was given an injection of 300,000 counts for vitamin D insufficiency , 3 days ago .

My 3 months glucose = 6.0.

Kindly review the x Ray report attached , might give more information about my case .

Hello and thanks for this information.
It takes a while for the vitamin D levels to improve symptoms. I still think you should be taking at least 1000 IU of vitamin d supplement daily until you see that your levels have improved to normal.
Stopping smoking could also be very helpful since smoke has lots of toxins and puts you at risk for multiple cancers.
Internal medicine doctors are not gastroenterologists. Since many of your symptoms have to do with your digestive system I think you need to see a gastroenterology specialist. You may need an upper endoscopy or colonoscopy which your internist cannot do.
I do not see an xray report. If you would resend that to me I would be happy to look at that. The only imaging I saw was an ultrasound.
