Brief Answer:
Ecg needs to be repeated

Detailed Answer:
Hello,
There is extreme right axis deviation and is likely due to lead reversal in arms. Also, there are q waves in v1-2 and poor r wave progression . And there are no significant St t abnormalities seen in precordial leads.
So, firstly Ecg should be repeated with correct lead position in arms and patient will need echo to rule out possibility of old asymptomatic anterior wall ischemia.
Hope this helps you and get back if you have any doubts.
