
Dear Sir/Madam, My Relative Is Suffering From Breakthrough Bleeding Which

My relative is suffering from breakthrough bleeding which is brown and reddish in colour.
She says that this sometimes happens whenever she drinks cold drinks and has happened in the past over 30 times. What could be the cause of the breakthrough bleeding?
She has a slightly underachieve thyroid with a reading of 62 versus upper limit 40. She is taking Oroxine 50mcg five times a week and started about two weeks ago.
Normally her cycle is regular but recently she had a delayed cycle of 7 weeks.
She did a thin prep test for NCSP HPV & Cytology (CVX-0) and these came back negative for HPV 16, 18 as well as others. However, she mentioned that after having this smear test she also experienced brown discharge.
Her urine was normal (UMM)
XXXXXXX for (CHM-)) Chlamydia and Gonorrhea were negative
She has yet to have blood tests for Prolactin SHBG LH FSH, free and total testosterone and BHCG.
I have also attached her ultrasound for the cervix report.
What could be the cause for the breakthrough bleeding and is any further testing required?

I have seen the attached report. Since all the tests are normal the bleeding that she is having is not related to vaginal infections or cervicitis. This is likely to be related to hormonal disturbances. The delayed cycles can also result in such abnormal bleeding. Usually such bleeding gradually stops on its own in few days. In case the bleeding is heavy then regular hormonal pills may be needed to regularise the periods. Further hormonal tests may be helpful.

Noted regarding the cause as likely hormonal imbalances and will follow up with blood tests.
However, does taking cold drinks and having the discharge and bleeding indicate any cause as bleeding after taking cold drinks has happened over 30 times previously.
Does having the thin prep test with smear and then experiencing the brownish discharge indicate weakness in the tissue or lining? It was not a Pap smear test but just a usual smear test.

The bleeding after taking cold drinks can be co incidental. It does not suggest anything regarding the cause. It is normal to have a slight bleeding after a thin prep test as there can be a mild trauma during the procedure. This can also suggest a mild inflammation in the area.
