Question: Dear Sir,
My aunt is suffering from fever. Everyday fever is coming along with shivering . Already blood sample has been given for testing of multiple Myeloma , FISH Studies, beta2 microglobin , LDH and Cytogenetic test, It will take around 5-6 days for blood test report to come. X-ray of bone was done. Only very few black spots on skull was present and rest of the bones are healthy. She is suffering from fever for last 3 months.
1) We are fearing about her health before treatment begins. Is fever a cause of concern? Is it life threatening? Because it will take around 6 days for report to come.
2) Please prescribe some medicine for her fever.
3)Is multiple myeloma on skull affect the prognosis?
4) What will be the cost of treatment?
Please reply as quick as possible.

Brief Answer:
Fever may be due to an infection
Detailed Answer:
Hi
Thanks for writing back.
Fever may be due to an infection resulting from poor immunity due to myeloma. It is a cause for concern. Appropriate cultures like blood and urine are taken and antibiotics usually administered for it. I am afraid I can't prescribe any medicines here. Please bring this into notice of her doctor.
No, multiple myeloma on skull will not change the prognosis. It depends upon the fish studies, beta 2 microglobulin, etc.
Cost varies widely between centers and regime chosen. It can be free in govt setup too. And as treatment is lifelong it will be very difficult to predict the total costs.
Hope this helps.
Regards

Brief Answer:
It is incurable disease and needs lifelong treatment
Detailed Answer:
Standard protocol is 4-5 months of initial therapy with combination of injections and oral medicines followed by autologous stem cell transplant followed by maintenance oral therapy again. When disease comes back again then next line of treatment started. Thus it becomes lifelong treatment.
At XXXXXXX most of the treatment is free. The initial therapy plus transplant may cost 50000-100000 approx.

Bone Marrow Aspirate Exam
Myeloblasts 2% ,Myelocyte !0%, Meta Myelocyte 7%, Plasma cells 50%
Impression- Plasma cell myeloma
Comprehensive Myeloma Protein:
Kappa Monoclonal gammapathy
blood kappa lambda ratio: 4
M spike-- 2.3g/dl
Beta2 microglobulin-- 3.17
Within 2 days LDH and FISH report will come.
1) What is the stage and Prognosis?
2) Can we expect survival for more than 10 years?
3) What should be the treatment options?
Her fever is gradually receding and sometimes shivering comes.
PLease reply.

Brief Answer:
Stage not final yet
Detailed Answer:
Stage will be 1-2 depending on serum albumin, fish and ldh reports. It won't be 3.
Definitely survival >10 yr is possible but that is not the average which is around 4 yrs. But that means 50% will survive more than 4 years.
Treatment is usually combination regime of anti myeloma drugs like bortezomib, dexamethasone, lenalidomide, thalidomide, etc for 4-6 months followed by high dose chemotherapy with autologous stem cell transplant.

Age-45, M spike-2.3, Albumin-2.7, Beta2 Microglobin - 3.17, LDH-120 , No Abnormality in FISH Study, Plasma cells-- 50% .
Currently she has fever and gradually receding . Doctor is saying if fever is controlled then treatment will be started.
Can we expect good prognosis... means > 10 years survival.
Whether low albumin level cause for concern?
Whether anti-myeloma drugs will be taken everyday or weekly?
Whether continuous hospital admission is necessary for treatment?
Thank You

Brief Answer:
Yes, after fever controlled treatment can be started
Detailed Answer:
Low albumin makes it stage 2 by the ISS system. Survival can be more than 10 years but that is not the average.,.Average is about 4 yrs...
Treatment is usually weekly but some are given continuously like Thalidomide and Lenalidomide.
Hospital stay is not required generally.
