Question: Dear Sir
My dad is 71 . He is very fit. Can walk 5-7 Kms plus every day and is very active. He doesn't have any diseases BP, Sugar etc.
His BP has always been 110/75 to 120/75 mark and we check vitals every month and ECG and all blood work
But when today we went to check whther he had Anal Fissures his BP was between 165/80 to 155/85 with high Systolic. We got a ECG done and it was normal. He had no symptoms
When we tested again after coming back home it was back to 120/75 , We checked again in evening and it was again normal at 120 / 80.
He has a very lean figure with NO Fat. His weight is 52 Kgs and 5ft 6 inches height
He has 2-3 ECGs every year and 1 ECHO as a precaution. which always has been normal
I am just wondering why was his BP high in Hospital
1. Was it due to White Collar BP ?
2. He was a bit stressed when he went to show the surgeon his Anal Fissure in Hospital.

Raised BP was due to Anxiety

Hi
As per your description,your father's BP was normal most of the times.
Raised BP in that one recording could be due to Anxiety.
That's why we ask the patients to relax for 10 minutes before recording BP in the Hospital.
Anal Fissure is a painful condition.I have seen people who are afraid to go to Bathroom.Per rectal examination leads to Severe pain in some.Any instrumentation like Proctoscope will also cause pain.Your father might have got tensed keeping these in mind.As the BP.is normal now, nothing to worry.
I have seen many patients,even very young whose BP recorded 170/100 in the Operation theatre before surgery.After few minutes of counselling,it will come to normal.Sometimes,we give Anxiolytics to relieve the Stress.
Then,the next recordings after surgery will be normal.
For Anal Fissure,diet is very important.He should take high fiber diet,He should drink plenty of liquids and eat fresh fruits and vegetables.Carbonated drinks,Non vegetarian should be avoided.Spicy food should also be avoided.If his stool is soft,Fissure heals quickly.If there is no response with dietary modifications,I prefer a simple surgery called Lateral Sphincterotomy.Before surgery,I also try with an Ointment containing Diltiazem which gave excellent results in few patients.

Thanks Dr. Very nice and detailed reply.
Just one more observation , we took his BP 3 times in hospital (as it was high), every time it was between 155-77 to 165-85

But after coming back home it was
1. 110-65
2. 125-80
3. 130-77


taken 3 times.
As you see , it might be WHITE COLLAR hypertension and anxiety.

He is being treated for Anal fissures now.
