Question: Dear Sir or Madame

About three weeks ago I was feeling a little under the weather with stuffy nose scratchy throat. I thought It was just my allergies. About a week later I developed a sore throat, with bright red tonsils. A week ago today I woke up and had chest congestion and a bad cough.

I could not get into my primary care provider so I went to an urgent care clinic. The doctor prescribed me cough medicine and an antibiotic Augmentin. I went home and started taking both not getting any better.

Last Saturday when I went to take my antibiotic I looked and didn’t realize that I had inadvertently been taking the wrong prescription. I was mistakenly taking penicillin 500V that was issued by my dentist.

I continue to take the penicillin as I had a full course of it, but by Sunday night my cough had gotten worse and I had a really high fever of 102.2. when I went to sleep. I woke up at 1:00 am coughing and feeling as though I was going to vomit. I ended up coughing so hard I passed out on my bathroom floor. When I came to my wife took my temperature and it was 103.8.

I went to the emergency room and they gave me fluids prednisone and the breathing treatment. I told the doctor what happened regarding taking the wrong antibiotic and she told me to not take the penicillin anymore and take the Augmentin.

I felt a little better Monday but by Tuesday night I was pretty sick again couldn’t sleep cough low-grade fever, my sore throat returned with white patches on my tonsils. I had night sweats so bad all of my bedding was soaked through.

I I was finally able to get to my general provider on Wednesday, yesterday. After hearing my symptoms reviewing the tests that were performed on me in the emergency room and doing a physical exam: he felt that the most likely cause for my sickness was mononucleosis. This would make sense as I do not remember having it as a child or teen.

He advised for a week off work, prescribed prednisone and a cough syrup. He also said to quit taking the antibiotics as this is probably viral. Yellow tinge mucus from nose and cough/ chest.

When I left his office I realized I forgot to tell him how I inadvertently started taking the wrong antibiotics to begin with. He also did not do a blood test for mononucleosis.

I would like your opinion if you feel this is mononucleosis or something else? Also do you feel I should stop my antibiotics? The last thing I want is a sinus infection when this is all over. Not sure if the mix up effects anything.

Thanks, I know I typed a long message, but felt it was important that you have all of the information.

Brief Answer:
Hii. Its evident from your history that you are having acute rhinipharyngit

Detailed Answer:
Hii. Its evident from your history that you are having acute rhinipharyngitis preferably acute tonsillitis. This may be due to any bacterial infection. So you have to give a routine blood including total count. Do a chest x ray and x ray sinus to see whether there is any congestion in the chest and infection in the sinus. Take a throat swab and sent for culture and sensitivity to the laboratory. If you are not allergic to augmentin or ceftum you can start with those antibiotic. Take antiallergic levocetrizine tablet. Take betadine gargle. Take steam inhalation. You can start saline nasal spray to clear your sinus. It will atleast 1 week to resolve. It may be any bacterial infection or diphtheria or anything viral infection. Take paracetamol to decrease pain and temperature. Continue taking oral food preferably more juice, soup, liquids. You have to follow antibiotic according to the laboratory report. Please review if you have any queries.

Detailed Answer:
Hii. Total count is high. You are having some sort of infection. Take a x ray chest and x ray paranasal sinus. Send a picture of tonsil and back of throat. Go to a clinic and give throat swab from the back of throat and send for culture and drug sensitivity report. Send the report. I would start tab augmentin, antiallergic tablet levocetrizine, steam inhalation through nose. Continue betadine gargle and warm saline gargle. Start nasoclear nose drop or saline nasal spray. Take more warm fluids and continue oral food.
