Brief Answer:
Is it severe and persistent?
Detailed Answer:
Hi,
If the pain is very severe and persistent, it should be addressed immediately at the nearest emergency centre (hospital).
But if the pain is not very bad, I would like to know a few details.
1. How did the pain start - did it start abruptly or was it gradual?
2. Is the pain worsening or improving since the start?
3. Can you describe the pain - dull ache / crampy?
4. Does it hurt on the right side too?

No sir only in left side with swelling with pain at moment of bike drive or wear of tite pant
Brief Answer:
Physical evaluation a must....
Detailed Answer:
Hi,
It would have been good if you had answered all my questions. Nevertheless you say that the pain is on the left alone and there is notable swelling in that region and it hurts to wear tight pant / sit on a bike.
This type of pain could be related to muscular strains or inflammation of testis (orchitis). Other rare possibility that I can now think of is varicocele.
With minimal information and without physical examination, it is not easy to confirm the nature / cause of pain and whether or it needs intervention. I will strongly recommend you visit a nearby surgeon for careful clinical examination. If necessary a scrotal ultrasound scan and urine test will be ordered.
In the meanwhile avoid tight clothing. Loose undergarments to support scrotal will help. You may also consider antinflammatory drugs like aceclofenac for pain relief.
Hope this helps. Let me know if you need any more help.
