Dear Sir I Suffered From Oral Lichen Planus From 2013

Question: Dear sir

I suffered from oral lichen planus from 2013 till 2015..I took homeopathy and I was disease free for 4 yrs..In November 2019 in developed a white Striae with red brown inflammation which look like inflammed blood vessels near lower left teeth..I started homeopathy again with lycored becosule and vitamin A to Z..Kindly have a look at inflammation spot please and advise..Is it oral LP spot with inflammation or normal cheek pigmentation?

I have chosen the speciality oral and maxillofacial surgery..Kindly ask HEALTH care magic to transfer question to the concerned department please..At the time of query name of Dr XXXXXXX XXXXXXX TS popped up who is an oral and maxillofacial surgeon. Thanks and Regards Ankur

Thanks for your query. I have gone through your query and the images. As per the images it is not looking like typical lichen planus. It can be a frictional keratosis with physiologic pigmentation commonly associated with sharp teeth or any rough surfaces over the teeth. Nothing to be panic. You need to consult an oral physician ( MDS in oral medicine and radiology) and get it evaluated. The medicines what you are taking will not cure your condition. Just based on the picture we cannot come to a diagnosis hence you need to consult a physician and get it evaluated. If it is a frictional keratosis then the possible causes like sharp teeth or rough surfaces should be rounded off. If it is a lichen planus, it needs to be treated with topical steroids.

Thanks for your reply. The pigmentation over the mucosa can be because of many reasons like normal physiologic pigmentation, smoking, pan chewing, lichen planus etc. So if you have the habit of using the above said things you need to quit the habit. Right now nothing to worry as such. For confirmation you need to consult dentist who is specialised in oral medicine and radiology.

I have consulted an MDS in prosthodontics and he said it's not due to sharp teeth as teeth are not touching that part of oral mucosa..In addition he said it might be normal cheek pigmentation and nothing to get worried of..He said dnt eat spicy food or smoke it might lead to inflammation or cluster of inflammed blood vessels...Stop pan masala or smoking and spicy foods..He prescribed lycored vitamin A to Z and kenacort for 15 days.. I am not using kenacort.. any advises please Regards

Thanks for your reply.
You are suppose to consult a dentist who is specialised in oral medicine and radiology. The possible causes for the white patch with pigmentation could be smoking, frictional keratosis, lichen planus or even in normal physiologic pigmentation. So if you have the habit of smoking or pan chewing you need to stop the habit. The image what you have uploaded is not looking like lichen planus. Nothing to worry. If you have burning sensation you can start with topical steroids after consulting the physician otherwise no treatment is required.
Reduce your stress level. Since stress can aggravate lichen planus.
