Detailed Answer:
Hi

Thanks for asking here

I have gone through the query and do understand your concern. I have seen the prescription but I am sorry to say that the image is blurred. So requesting you to reattach that again.
Most common cause of fever in this age is some viral infection and in that condition conservative treatment with paracetamol is sufficient. But I am more concerned because fever in most of the viral infection shows a declining trend after 72-96 hours. Here as fever is prolonged with some rashes also I would like to rule out infections like dengue, typhoid, malaria or some other bacterial infection. Based on clinical examination one can get a clue about the site/type of infection.
In view of prolonged fever I would like to get blood tests like complete blood count including platelet count, c reactive protein and procalcitonin(for bacterial infection),typhidot test for typhoid, malaria antigen test for malaria and also the dengue test.Further management will depend on the reports.
For fever continue giving paracetamol. if you have any doubt you can visit another pediatrician for second opinion.
I hope this helps.
Please let me know if you have any more doubts.
Regards
